How do I know if I’m eligible to get the vaccine?
You can find the latest news on who is eligible to receive the vaccine at http://ourshot.in.gov/.
How do I register to receive the vaccine?
If you are in the eligible group listed on ourshot.in.gov, you can register for your vaccine through https://vaccine.coronavirus.in.gov/ or by calling 2-1-1
What vaccine is given at the Family Health Clinic?
We administer the Moderna COVID-19 vaccine.
Is there a charge for the COVID-19 vaccine?
No! There is no charge for the vaccine. You can bring an insurance card that we can bill. But, whether you have insurance or you do not have insurance, there will be no charge.
What do I need to bring to the vaccination site?
You’ll bring a photo ID, preferably a drivers’ license or Indiana-issued ID. You can also bring your insurance card if you like.
What do I do once I arrive at the vaccination site?
You’ll park at the location and come in! Keep in mind, some vaccine sites are at a community location instead of at the clinic, but you will know this information when you register.
Once you come in, the greeter will guide you to the check-in. At check-in, they’ll process and register you. Then, you’ll be sent to get your vaccine. After your vaccine, you’ll go to an observation where you’ll wait for a little bit and then schedule your second vaccination!
Are there ever extra vaccines at the Family Health Clinic?
