Starting mid-January, the Family Health Clinic will be providing COVID-19 vaccines at all of our locations: Burlington, Delphi, Monon, and Wolcott.

Registering for a COVID-19 Vaccine at the Family Health Clinic
If you’re currently eligible to receive the vaccine, you can register to receive your vaccine by going to https://vaccine.coronavirus.in.gov/ or you can call 2-1-1 for help. If you’re not sure if you’re eligible, click here.
If you’d like to receive your shot at one of the Family Health Clinic’s locations, select your desired location during the registration process.
After registering, follow the directions and show up at your pre-registered time for your vaccine!
Do you have to register in advance?
Yes! Anyone seeking a COVID-19 vaccine must register through https://vaccine.coronavirus.in.gov/ or by calling 2-1-1.
We will not be accepting walk-in appointments. Appointments must be made in advance.
Who is able to get a COVID-19 vaccine?
As of 1/21/21, these Indiana residents are eligible to get the coronavirus vaccine:
- Healthcare workers or volunteers
- First responders
- People 70 years or older
