A Patient-Centered Medical Home is not a place; it’s a partnership with your primary care provider. As a PCMH, the Family Health Clinic puts you at the center of your care, making sure that your health is overseen before, during, and after your clinic visit. There are five main factors of a Patient Centered Medical Home like the Family Health Clinic.
The Five Factors of a Patient-Centered Medical Home
- Comprehensive Care | We at the Family Health Clinic are accountable for meeting a large majority of your physical and mental health care needs. This includes prevention and wellness, acute care, and chronic care. In order to provide comprehensive care, we have a team of care providers that serve you.
- Patient Centered | As a PCMH, we look to create a relationship with you and your family that understands and respects your unique needs, culture, values, and preferences.
- Coordinated | We help coordinate your care across elements of the broader health care system. This includes specialty care, hospitals, home health care, and community services and supports.
- Accesible | Our goal is to give you easily accessible services that have shorter waiting times for your urgent needs, enriching in-person visits, and around-the-clock telephone or electronic access to one of our providers, and other methods of communication such as email and telephone care.
- Quality and Safety | As a PCMH, we have a commitment to quality care and quality improvement by using evidence-based medicine and clinical decision-support tools to guide shared decision making with you and your family, engaging in performance measurement and improvement, measuring and responding to patient experiences and patient satisfaction, and practicing population health management.
Participating In Your Care
We want you to become an active participant in your health care. Here are a few ways to make the most of the Family Health Clinic as a Patient-Centered Medical Home.
- Make sure you understand your diagnosis or condition by asking the Family Health Clinic staff any questions you have
- Provide us with your most complete and accurate medical history at each visit, so we can make informed decisions together
- Work with us to create healthcare goals and track them
- Prepare questions you may have about your health ahead of your visit
- Know a list of your medicines, vitamins, and other supplements or home remedies before your visit
- Keep us informed on your visits to other medical care providers
