The Family Health Clinic Patient Portal
The Patient Portal empowers you to take better control of your health and wellness by giving you access to your medical information. Here are some of the great benefits of our new Patient Portal:

- Secure Messaging: By using the Secure Messaging service included with the Patient Portal, you can now send private messages to your primary care provider at the Family Health Clinics with information or inquiries.

- Request Appointments: Request appointments through the Patient Portal
- Request Rescheduling: If you run into a scheduling conflict, you can request that your appointment be rescheduled.
Here is some even more exciting news; this just the beginning! We plan to provide even more services through the Patient Portal in the future.
The Family Health Clinic Texting Messaging Services
We are also happy to announce that Text Messaging Services will also be available to patients with a mobile device. We know that texting is a normal way for people to stay in touch, and sometimes it’s even the preferred way! For that reason, we have started our text messaging services as a way to keep in touch with you, too.
With our Text Messaging Service, we can:

- Send you reminders of upcoming appointments

- Inform you of activities you may find important in promoting your health as well as your wellness
- Remind you of the need to schedule annual exams, pediatric wellness visits, and many other reminders that help you manage your health.

Call (800) 321-5043 for more information. If you’re at the clinic, ask any of the staff to help you register for an account on the Patient Portal. Once registered, you will always find the Patient Portal at http://NCNCMyChart.org.
Want to connect with Text Messaging?
Again, you can call and ask about this new service, or talk to one of our staff members and they will help you. Our staff will verify your phone number and email address and set you up to receive Text Messages.
